HFEA Consent Forms

Prior to treatment, patients are asked to complete certain consent forms. Below is a list of the most commonly used HFEA consent forms. We have provided these so that patients can familiarise themselves with the forms prior to completion. At the appropriate time during your treatment journey, GCRM staff will instruct you on which forms are relevant to your own particular treatment.
